This consent form relates to clinical photography taken before, during, or after your treatment at Be Elite Aesthetics.

Clinical photographs form an important part of your medical record. They help your practitioner track treatment progress, plan future sessions, and maintain an accurate visual record. Photographs are taken in a standardised manner with consistent lighting and positioning.

Photographs taken for your medical record are processed under Article 9(2)(h) UK GDPR (health care purposes). Any use beyond your medical record (such as marketing, social media, education, or publication) requires your separate, explicit consent and is processed under Article 6(1)(a) UK GDPR (consent). You may consent to clinical record photography without consenting to any other use.

Patient Declaration

By signing this consent form, I confirm that:

1I consent to clinical photographs being taken before, during, and/or after my treatment and stored as part of my confidential medical record.
2I understand that clinical photographs will be stored securely, accessible only to my treatment team and authorised clinical staff, and retained in accordance with Be Elite's Data Retention Policy (8 years from the date of last treatment).
3I understand that I may request to view, receive copies of, or request deletion of my clinical photographs at any time, subject to applicable record-keeping obligations and retention periods.
4I understand that my clinical photographs will NOT be used for marketing, social media, educational, or any other non-clinical purpose without my separate, explicit, written consent.
5I understand that if I do provide separate consent for non-clinical use, I may withdraw that consent at any time by contacting the clinic, and that previously published material will be removed where reasonably practicable.
